Supporting Veterans With Expert Documentation for VA Disability Claims
At Freeman Wellness & Aesthetics, we recognize the challenges many veterans face when navigating the VA disability process. While the VA makes the final decision on disability ratings, the strength of your medical evidence you submit can make all the difference. We take the time to thoroughly document your service history, health concerns, and daily limitations — details that can sometimes feel overlooked or rushed in standard evaluations. Our goal is to ensure your service history, health concerns, and limitations are thoroughly documented and not overlooked.
We provide:
Why Veterans Choose Us 🇺🇸
Freeman Wellness & Aesthetics is not affiliated with or endorsed by the U.S. Department of Veterans Affairs. We do not make disability determinations. Our role is to provide independent medical evaluations and supporting documentation that veterans may submit as evidence in their VA claims.
At Freeman Wellness & Aesthetics, we believe in making this process simple and transparent.
We charge a flat fee of $800, which includes everything you need for a thorough, credible disability evaluation.
What’s Included:
Most veterans need the full package — evaluation, DBQs, and Nexus Letter — not just one piece. By offering a flat fee, we eliminate confusion and ensure you receive everything necessary to present the strongest claim possible.
For $800, you receive a comprehensive evaluation and documentation set that typically costs $1,200–$1,500 elsewhere. *Additional DBQs are available at $50 each, so your cost only increases if you are filing multiple conditions beyond the standard two.

What is included in the $800 flat fee?
Your fee includes:
If you need more than 2 DBQs, each additional DBQ is $50.
Example: If you require 4 DBQs, your fee would be $800 + $100 (2 additional DBQs) = $900 total.
What is a DBQ?
A Disability Benefits Questionnaire (DBQ) is an official VA form that allows a licensed provider to document your symptoms, diagnoses, and functional limitations in a structured format. DBQs are designed to align with the VA’s rating criteria, making them a critical piece of supporting evidence for your claim.
What is a Nexus Letter?
A Nexus Letter is a medical opinion written by a licensed provider that explains whether your current condition is connected to your military service. When supported, the letter uses the VA’s required language: “at least as likely as not (50% probability or greater).” A well-written Nexus Letter can make the difference in whether a claim is approved.
When do veterans use this service?
Veterans seek our evaluations at different stages of the disability process:
Important Note: All claims and appeals must still go through the U.S. Department of Veterans Affairs, which is the only authority that can approve benefits and assign ratings. We cannot guarantee approval or a specific rating. Our role is to provide clear, objective, and comprehensive medical evidence that veterans can use to support their VA claim at any stage.
Can this replace my VA Compensation & Pension (C&P) exam?
No. The VA may still require you to attend a C&P exam. Our evaluation is independent and designed to provide additional supporting evidence that you can submit with your claim or appeal.
Do you prescribe medications at this visit?
No. This visit is strictly for documentation purposes. Medications are not prescribed and no ongoing treatment is provided during disability evaluations.
Do you guarantee I will be approved for benefits?
No. Only the VA can make decisions on disability ratings and approvals. Our role is to provide objective, credible medical documentation to strengthen your case.
How many DBQs will I need?
Most veterans require 1–3 DBQs. Common examples include back/spine conditions, knee or joint issues, migraines, sleep apnea, PTSD, or tinnitus. If you need more than 2 DBQs, additional forms are $50 each.
Example: 2 DBQs are included in the $800 flat fee. If you need 5 DBQs, your total would be $800 + $150 (3 extra DBQs) = $950.
How long does the process take?
Do I need to live in North Carolina?
You do not need to be a resident, but you must be physically located in North Carolina at the time of your telehealth appointment (per licensing laws). Veterans from other states are welcome to travel into NC for their evaluation.
Do you accept insurance?
No. These evaluations are cash-pay only, since insurance does not cover DBQs or Nexus Letters. Payment is due at the time of service.
Will I receive copies of everything?
Yes. You will receive signed copies of your DBQs, Nexus Letter, and a summary of findings with clear instructions on how to submit them to the VA.
