Patients wishing to establish care for routine primary care visits, medication management, chronic disease management, labs, tests/imaging.

(Visit time: 45 minutes - 1 hour)

Routine follow-up visits for established patients & medication refills. 

(Visit time: 20 - 30 minutes) 

*Patients who have not been seen in greater than 13 months will require a new initial visit. 

*For Established Patients Only* 

Refills for routine medications. 

• 1-3 Medications                                                $15.00
• 4-7 Medications                                                $30.00
• 8-10 Medications                                              $40.00

• Greater than 10 medication refills will require a Follow-visit. 

*Established patients must have been seen for an initial or follow up visit within 1 year. 

Patients who have established care with Freeman Wellness & Aesthetics in the past and have been seen within 1 year. 

(Visit time: 20 - 30 minutes) 

Patients who have not established care with Freeman Wellness & Aesthetics. 

(Visit time: 20 - 30 minutes) 

Established or Non-Established patient who have been seen within 1-14 days for a past sick visit with our office. 

(Visit time: 15 - 25 minutes)
